This is the procedure to issue a "Physical Disability Certificate," which is required for individuals with permanent physical disabilities due to illness or injury to use various welfare systems.
Certificates from Grade 1 to 6 are issued depending on the degree of disability (Note: Grade 7 is not eligible).
