Bring the hospital's Birth Certificate and your Boshi-techo to City Hall within 14 days of birth. Don't forget to apply for Child Allowance and Free Medical Care at the same time!

For fraud, malicious sales, or suspicious contracts, consult the Ota City Consumer Affairs Center (0276-47-1111) or the Consumer Hotline '188' as soon as possible.

Bring the death registration (death certificate) to City Hall, an Admin Center, or a Service Center. Advance reservations are required if using the city crematorium.

Bring your photo ID and seal (optional) to the Citizens' Affairs Division on the 1st floor of City Hall. An attachment is required if you have minor children.

To help bereaved families smoothly navigate the various administrative procedures, Ota City provides the 'Okuyami Handbook' and the 'Okuyami Procedure Navigator'.

Ota City residents can book a free 20-minute consultation with a lawyer (up to twice a fiscal year) for advice strictly on legal interpretations.

Submit your move-in notification within 14 days of moving to Ota City. Foreign residents must apply at the main City Hall or Yabuzukahonmachi Office.

Submit your move-out notification between 14 days before and 14 days after moving. You will receive a move-out certificate required for moving into your new address.

My Number Cards are issued free of charge. Various procedures can be done at City Hall or Service Centers, but advance reservations may be required for weekends, nights, or receiving the card.

The Environmental Measures Division handles complaints about noise or odors, but cannot intervene in civil disputes between individuals. It is recommended to calmly discuss the issue with the other party first.

Do not try to solve online troubles or dark part-time job damages alone. Consult the Consumer Affairs Center (0276-47-1766) or police (#9110).

Available at the counter, by mail, or at convenience stores. Note: If a proxy requests a Resident Record with a My Number, it will be mailed to the applicant's home instead of handed to the proxy.

Residents aged 15+ can register a seal. The fee is 200 yen. Registration can be completed on the same day if you bring a valid photo ID.
